We keep this simple and collect as little as possible.

What we collect

The questionnaire asks for business information about your practice, such as its name, address, and the names of your Privacy and Security Officers. It never asks for patient information. Please do not enter any patient information.

Where it is stored

We do not run a customer database. When you check out, your answers are stored with your order in our payment processor, Stripe, so your download link keeps working. Stripe processes your payment details; we never see or store your card number.

How it is used

Your answers are used only to generate your documents and to support your order. We do not sell or share your information, and we do not use it for advertising.

Your choices

To ask us to delete your order information, email contact@eaglizerit.com.
